| Dr. Baldwin is a Plastic and Reconstructive Surgeon practicing at Clarian Arnett Health located at 1500 Salem St. in Lafayette. He has practiced here since 2002, after moving from Salt Lake City, Utah. He has practiced Plastic Surgery since 1984, and is board certified. Our facility houses two Operating rooms, two exam rooms, a procedure room and a recovery room. We also have a cozy waiting room, canteen and hospitality room with a TV. Dr. Baldwin and staff wish to provide our patients with the best medical care and a relaxing environment. We take our time and try our best to make each experience a pleasant one.  James D. (Sam) Baldwin, MD
Dr. Baldwin specializes in cosmetic surgery, is certified by the American Board of Plastic Surgery and is a member of the American Society of Plastic Surgeons. After graduating from the University of Utah School of Medicine, Dr. Baldwin completed residencies in general surgery at the University of Rochester (New York) and plastic surgery at the University of Utah Medical Center. He is a former Clinical Associate Professor of Surgery at the University of Utah. In 2002 Dr. Baldwin joined Clarian Arnett Health and is on the medical staff at both St. Elizabeth Hospital and Home Hospital. He is a staff surgeon with Operation Smile. He and his wife Cindy are the parents of four children.
